Abstract:

In our days, the patient demands and expectations are very high. Restoring only the clinical aspect of the tooth is no longer an option; therefore the clinician must consider surgical procedures to restore the periodontal aesthetic balance as well. The aim of this study is to evaluate the possibilities of restoring the aesthetic parameters of the dental gingival area using 2 methods: papillary regeneration and root covering techniques. We had a sample of 134 psychiatric patients divided in 2 subgroups according to the procedure performed: 62 patients with papillary restoration using the connective tissue graft technique and 72 patients who undergone treatment for gum recession using the coronal repositioned flap technique. We assessed the aesthetic outcome at 1 month, 6 months, 12 months and 18 months after the procedure related to the initial situation. Besides the clinical evaluation we did also a subjective evaluation, investigating the level of patient satisfaction. The parameters evaluated were: degree of root coverage, location of the muco-gingival line (for root covering technique) and degree of papilla restoration, colour match of the tissue and soft tissue appearance for the papillary reconstruction technique. The study showed that the aesthetic outcome of the root coverage technique is more predictable than the one of the papillary reconstruction. Regarding the patient satisfaction, the papillary reconstruction seems to be less successful then root coverage techniques. A key factor in all cases was patient compliancy to the treatment, postoperative indications and periodical revision. Also the good clinical evaluation prior to the procedure and the well documented treatment plan allow an accurate prediction of the result.
